Incident Review Training

A good incident review does more than establish what happened.

Our practical training helps health and social care professionals understand how and why an incident occurred, explore contributory and system factors, involve those affected, and turn learning into meaningful improvement.

The programme combines a structured approach to incident review with practical exercises, case discussion and systems thinking, helping participants build the confidence to undertake reviews that are proportionate, thorough and focused on learning.

Participants are also supported with access to the CLS Educate App and CLS Projects resource website, providing practical tools and resources to support their work beyond the training room.

SWARM Huddles 

Swarm huddles provide a rapid, structured way for teams to come together soon after an incident, while events are still fresh.

The focus is on understanding what happened, identifying the factors that influenced the event, capturing immediate learning and deciding what needs to happen next — without defaulting straight to a full review.

Our practical half-day programme gives participants the confidence and tools to plan, facilitate and use Swarm huddles effectively within health and social care settings.

Incident Review Mentoring and Clinics

Incident review can be complex, especially when staff apply new approaches or handle challenging cases.

Our Incident Review Mentoring and Clinics provide practical support for professionals and managers who lead, facilitate or contribute to incident reviews.

Through one-to-one mentoring, small-group support and facilitated clinics, participants can discuss anonymised cases, work through challenges and strengthen their approach in practice.

The aim is to provide a practical space for reflection, problem-solving and continued development — helping investigators build confidence, judgement and consistency over time
